Outcomes based care planning

In conjunction with the University of Kent and NHS England, we are the first homecare provider to integrate the Adult Social Care Outcomes Toolkit (ASCOT) to truly deliver outcome-focused care.

As we get to know the person we support and clearly define their chosen outcomes under the 7 ASCOT domains, we break these down into daily goals to be completed with their wellbeing support worker and track these on a visit-by-visit basis.

Each visit feeds back into the person's outcomes journey, with activities completed, notes, images and any incidents recorded chronologically, creating a transparent and shared record across the team.

Regular face-to-face ASCOT reviews capture how the person feels about their quality of life, generating domain scores that show progress over time, and allowing us to measure the tangible difference BelleVie makes.

Buurtzorg model

The BelleVie approach to care is based on the Buurtzorg model, a community nursing style that began in the Netherlands in 2006. It was founded by Jos de Blok as an alternative to traditional, task-focused home care. Key principles include:

  • Small, self-managing teams: Teams of about 10-12 carers organise their own work without traditional managers.

  • Holistic care: carers assess the person's overall physical, psychological, and social needs.

  • Relationship-based care: people are cared for by a small number of familiar carers, promoting trust and continuity.

  • Promoting independence: helping people regain or maintain the ability to care for themselves, reducing long-term dependence on services.

  • Community connections: carers work closely with families, neighbours, volunteers, GPs, therapists, and social services to build a support network around the person.

  • Minimal bureaucracy: documentation is streamlined so carers spend more time providing care and less time on administrative tasks.

Wellbeing OS

No existing software fits the Buurtzorg model of self-managing care teams. So in order to move away from traditional time and task models, BelleVie has co-designed its own proprietary Wellbeing Operating System (Wellbeing OS) with its care teams.

This comprises of Team Hub, the web based admin platform, and the BelleVie Way app for wellbeing support workers. Both are designed to reduce administration so carers can spend more time providing care rather than completing paperwork.

These digital tools enable self-managing care teams to work without a traditional line manager. Care workers can create and manage their own rotas, swap or request cover for shifts, and coordinate with neighbouring teams. Team Hub provides shared information about the people they support, including care notes, wellbeing updates, and care plans, and lets teams track and manage their own workloads, travel time, and visit schedules, with route planning integrated into mapping tools.

Skills for Care

Skills for Care is an independent charity and the strategic workforce development body for adult social care in England. They partner with the Department of Health and Social Care, employers, and care workers to provide tools, resources, and intelligence for recruitment, training, and workforce retention.

BelleVie's implementation of the Buurtzorg model and Wellbeing OS is now being shared by Skills for Care across the country. They provide a toolkit of resources based on BelleVie's approach, designed to support transformation across adult social care and show others in the sector how to introduce self-managing teams, relationships and community at the centre.
